CPT 94762 (oximetry) is covered when intermittent arterial blood gas sampling may miss important variations or when hypoxemia requires assessment of supplemental oxygen needs or therapeutic regimen. CPT 31720 is payable only when personally performed by a physician or qualified NPP and is generally allowed for up to 3 consecutive days or 3 identical services in 30 days, with additional payments requiring individual review and documented medical necessity; CPT 94760–94762 are bundled and not separately payable with critical care codes 99291/99292.
