Billing and Coding: Intravesical Instillation of Bacillus Calmette-Guérin (BCG)

Effective 2019-07-01 Medicare requires use of HCPCS J9030 (BCG live intravesical instillation, 1 mg) and billing with units equal to the milligrams actually instilled. J9031 is no longer payable; when doses are reduced from the 81 mg FDA label because of BCG shortage, report the actual mg administered and document dose and clinical justification in the medical record.
