Coverage for transthoracic echocardiography (TTE) requires medical record documentation demonstrating medical necessity consistent with the LCD; each service must have a formal written report, image documentation, and required measurements. Contrast echocardiography is covered only when conventional studies are inconclusive and repeat studies are allowed to monitor changes when clinical status changes or disease progression is suspected; claims without valid ICD-10 codes, required referring physician/NPI, or adequate documentation may be returned or denied. Frequency is guided by clinical circumstance and the LCD, with Emergency Room services excluded from frequency limits.
