Coverage for 25-hydroxy vitamin D testing (including fractions) follows the reasonable and necessary criteria set forth in LCD L33771; perform testing only when supported by the LCD. Claims must not be submitted for services not covered by the LCD (use the appropriate modifier for non-covered services), and the medical record must fully document patient identification, provider signature, support for ICD-10 and CPT/HCPCS coding, test results, and the resulting treatment plan.
