Billing and Coding: RAST Type Tests

RAST type tests are covered when medically necessary and supported by the related LCD and current standards of practice. Claims must include valid ICD-10-CM code(s), and when applicable the referring/ordering physician's name and NPI; complete medical record documentation (history, exam, and test results) must be retained and made available to Medicare. Procedure codes may be subject to NCCI or OPPS edits, and services performed in excess of established parameters may be reviewed for medical necessity.
