Billing and Coding: MolDX: Molecular Diagnostic Tests (MDT)

This policy specifies billing and coding requirements for MolDX Molecular Diagnostic Tests: report the appropriate CPT code, submit exactly 1 unit of service, and include the appropriate DEX Z-Code identifier adjacent to the CPT code in prescribed claim fields. DEX Z-Code placement differs by claim type (Part B electronic: Loop 2400 or SV101-7; Part B paper: Box 19; Part A electronic: SV202-7; Part A UB04: Block 80) and the SV101-7 field must contain the DEX Z-Code with no additional characters.

"When entering the DEX Z-Code on the SV101-7 documentation field for Part B claims, do not add additional characters or other information on that line."
