This policy provides billing and claim‑field instructions for reporting the Envisia™ (Veracyte) Genomic Classifier under LCD L37857. Claims must include the appropriate CPT and ICD‑10‑CM codes, report one unit of service (1 UOS), and include the DEX Z‑Code™ identifier adjacent to the CPT code in the specified Part A or Part B claim fields.
