Coding guidance: use CPT 88271-88291 for ISH performed as an ancillary analysis to cytogenetic studies for oncologic or inherited disorders, and use CPT 88365-88377 when a pathologist performs ISH as an adjunct to surgical pathology or cytopathology (qualitative vs quantitative distinction). Report one unit of service per CPT code, include an appropriate ICD-10-CM diagnosis code, and note that a DEX Z-Code is optional but if provided must be placed adjacent to the CPT code in the specified claim fields.
