This billing and coding guidance requires reporting the appropriate CPT and ICD-10-CM codes, entering 1 unit of service, and including the appropriate DEX Z-Code adjacent to the CPT code in specified comment/narrative fields for Part A and Part B claims (Loop 2400/SV101-7/Box 19 for Part B; SV202-7/Block 80 for Part A). Do not add additional characters or information on the SV101-7 line when entering the DEX Z-Code.
