Billing and Coding: MolDX: NRAS Genetic Testing

This billing/coding policy instructs providers to report NRAS Genetic Testing by selecting the appropriate CPT code and ICD-10-CM diagnosis code, and by entering 1 unit of service. The DEX Z-Code identifier must be included adjacent to the CPT code in the comment/narrative field on specific claim fields: for Part B (Loop 2400 or SV101-7 on 837P; Box 19 on paper) and for Part A (Line SV202-7 on 837I; Block 80 on UB04).
