This billing and coding guideline requires reporting the appropriate CPT code and entering one unit of service for Molecular Diagnostic Tests, with the DEX Z-Code identifier placed adjacent to the CPT code on specified claim fields. DEX Z-Code placement differs by claim type: Part B (Loop 2400 or SV101-7 for 837P; Box 19 for paper) and Part A (SV202-7 for 837I; Block 80 for UB04), and the SV101-7 entry must contain only the exact DEX Z-Code without extra characters.

"When entering the DEX Z-Code on the SV101-7 field for Part B claims, do not add any additional characters or other information on that line."
