Billing and Coding: MolDX: Envisia™, Veracyte™, Idiopathic Pulmonary Fibrosis Diagnostic Test

This policy provides billing and coding instructions for reporting the Envisia™ Genomic Classifier (Veracyte) and supplements MolDX LCD L37905. Claims must report the appropriate CPT code with 1 unit, include the DEX Z-Code™ identifier adjacent to the CPT line in specified comment/narrative fields for Part A and Part B electronic and paper claim formats, and include an appropriate ICD-10-CM diagnosis code supporting medical necessity.
