Unattended polysomnography services billed with HCPCS G0398/G0399/G0400 or CPT 95800/95801/95806 are covered only when the study meets the narrative definition for the specific code and is performed without a technologist present. G0400 requires use of a type IV portable monitor with at least three channels measuring respiratory movement, airflow, and oxygen saturation; documentation must demonstrate these device and recording criteria and comply with LCD L36593. CPT 95800 and 95801 are recognized for services with dates of service on or after 2011-01-01.
