Billing and Coding: MolDX: NRAS Genetic Testing

This billing/coding guidance complements MolDX LCD L35442 for NRAS genetic testing and requires reporting the appropriate CPT code and ICD-10-CM diagnosis code. Claims must list 1 unit of service per CPT line and include the appropriate DEX Z-Code identifier adjacent to the CPT code in the specified comment/narrative fields for Part A (837I/UB04) and Part B (837P/paper) claim types.

"NRAS genetic testing services (MolDX L35442) should be reported when performed using the appropriate CPT code for the NRAS test."
