Billing and Coding: ProMark® Risk Score

This billing/coding guideline requires reporting the ProMark® Risk Score assay with CPT code 81479 and selecting ICD-10-CM code C61 (prostate cancer). Claims must include 1 unit of service and the appropriate DEX Z-Code™ identifier adjacent to the CPT code in the specified Part A (Line SV202-7 / Block 80) or Part B (Loop 2400 / SV101-7 / Box 19) fields.
