Claims must include a valid ICD-10-CM diagnosis and required documentation (signed records, test recordings, physician SEP report with nerves/latencies), and ABN/modifier rules (GA/GX/GZ/GY and CMS‑R‑131) and provider credential documentation must be followed; services beyond established parameters may be reviewed or denied.
