Morton's neuroma injections should be billed specifically with CPT 64455 or 64632, while plantar fasciitis injections use CPT 20550 (ICD-10 M72.2) and tendon origin/insertions use CPT 20551; injections involving both the plantar fascia and calcaneal spur should be reported with a single 20551. Clinical documentation must justify the diagnosis and treatment decision and be available on request; more than three injections to the same site within six months require documented justification, and services lacking required documentation or medical necessity may be denied. Coding must also comply with CCI edits, which take precedence over this policy.
