Hypoglossal nerve stimulation procedures for obstructive sleep apnea should be reported using CPT codes 64582 (insertion), 64583 (revision/replacement), and 64584 (removal); CPT 64568 is no longer used for this technology effective 01/01/2022. Coverage is contingent on meeting the reasonable and necessary criteria in LCD L38387 and is subject to NCCI coding edits. Complete, legible medical records including provider signature and documentation supporting the selected diagnosis codes must be maintained and provided upon request.
