MolDX blood product molecular antigen typing panels 0001U and 0084U (germline tests) are covered but limited to one test per beneficiary lifetime. Individual antigen codes 0180U-0201U, 0221U, 0222U and CPT codes 81105-81112 are non-covered when billed alone and will only be considered when submitted as part of an approved panel; claims must include the appropriate CPT/PLA code, UOS, DEX Z-Code in designated Part A/B fields, and an ICD-10-CM diagnosis code.
