This policy provides billing and coding instructions for reporting the SelectMDx for Prostate cancer test; it requires submission of the appropriate CPT code, 1 unit of service, the appropriate DEX Z-Code identifier in specified comment/narrative fields for Part A and B claims, and an appropriate ICD-10-CM diagnosis code. Coverage determinations and medical necessity are governed by the referenced LCD DL38247.
