Billing and Coding: MolDX: Blood Product Molecular Antigen Typing

MolDX blood product molecular antigen typing panels 0001U and 0084U are covered as germline blood-product antigen tests when billed consistent with LCD L38249 and are limited to one test per beneficiary lifetime. Individual antigen codes (0180U–0201U, 0221U, 0222U, 81105–81112) are noncovered when billed alone and will be considered only as part of a comprehensive panel. Claims must include the appropriate CPT/PLA code, UOS, DEX Z-Code identifier in the specified claim fields, and an appropriate ICD-10-CM diagnosis code.
