Billing and Coding: MolDX: Pharmacogenomics Testing

This policy provides billing and coding instructions for reporting MolDX pharmacogenomics testing: submit the appropriate CPT code with 1 unit of service, report the applicable ICD-10-CM diagnosis code, and include the DEX Z-Code identifier adjacent to the CPT in the specified comment/narrative fields for Part A and Part B claim types. It supplements LCD DL38394 and does not define medical necessity or coverage determinations.
