Cardiovascular stress testing (exercise and/or pharmacologic, including stress echocardiography) is covered only when medically necessary and at an appropriate frequency — routine annual testing or routine post‑MI/CABG/PTCA follow‑up without symptoms or reassessment indications is not reasonable/necessary and duplicate testing must be justified. Coverage requires documentation of the clinical diagnosis and specific reason for the study (including why a stress echo vs electrical stress and why pharmacologic stress or concurrent nuclear testing are used), the referral order, interpretation of all test components, justification for test frequency, physician supervision documentation, and leasing contracts for leased staff when applicable.
