CT and MRI scans of the head and neck are covered when the medical record documents that the study is reasonable and necessary for diagnostic evaluation, including relevant history, physical exam, laboratory results, and signs or symptoms. Claims must include accurate HCPCS/CPT coding, comply with CCI edits and applicable LCD guidance, and supporting documentation must be available to Medicare upon request.
