Head and neck CT and MRI scans are covered when the medical record documents the test is reasonable and necessary. Claims must include accurate HCPCS/CPT coding (subject to CCI edits) and medical records (history/physical, labs, signs/symptoms) must support the need for the scan and be available to Medicare on request.
