This policy provides billing and coding guidance for GlycoMark testing for glycemic control. Claims for GlycoMark should use CPT 84378 (or 84999 when appropriate) and follow ABN modifier guidance: GX for voluntary ABNs and GY to indicate statutorily excluded services; a statutory exclusion does not require an ABN.
