This billing guideline requires reporting the appropriate CPT and ICD-10-CM codes and entering exactly 1 unit of service, with the DEX Z-Code identifier placed adjacent to the CPT code in the specified comment/narrative fields for Part A and Part B claim types. For Part B electronic claims use Loop 2400 or SV101-7 (5010A1 837P) or Item 19 for paper; for Part A use Line SV202-7 (837I) or Block 80 (UB-04); do not add extra characters to the SV101-7 DEX Z-Code field.
