Policy Summary
Medicare home health physical therapy is covered when the beneficiary is homebound, under a physician's care, receiving services under a physician-established and periodically reviewed plan of care, and needs skilled physical therapy documented as reasonable and necessary. Physical therapy must be ordered by a physician, provided by qualified PT personnel, and supported by a valid physician-signed plan of treatment and per-visit treatment notes; packaged programs and services by aides/technicians are not covered.
