Blood product molecular antigen typing is recognized as a germline test and comprehensive antigen panels (not individual antigen codes) are the appropriate mechanism for testing; panels 0001U and 0084U are allowable. Individual antigen codes 0180U-0201U, 0221U, 0222U, and 81105-81112 are noncovered when billed separately and will be considered only when submitted as part of a panel. Claims must include the CPT/PLA code, unit of service, the appropriate DEX Z‑Code identifier in the specified Part A/Part B claim fields, and an appropriate ICD-10-CM diagnosis code; panels 0001U and 0084U are limited to one lifetime use per beneficiary.
