Billing and Coding: MolDX: Envisia™, Veracyte™, Idiopathic Pulmonary Fibrosis Diagnostic Test

Billing for the Envisia Genomic Classifier requires reporting the appropriate CPT code with 1 unit of service, including the DEX Z-Code identifier adjacent to the CPT code in specified claim comment/narrative fields for Part A and Part B (electronic and paper) claims, and listing an appropriate ICD-10-CM diagnosis code. Claims must be submitted in accordance with the associated Local Coverage Determination.
