Transrectal ultrasound (TRUS) is covered when medically necessary and consistent with the related LCD and supported by medical record documentation. CPT code 76872 is limited to two occurrences per year; claims must include a valid ICD-10-CM diagnosis code and, when applicable, the referring/ordering physician's name and NPI, and are subject to NCCI and OPPS edits.
