This policy A57441 provides billing and coding guidance for MolDX HLA-DQB1*06:02 testing related to narcolepsy and is intended to be used with the associated Local Coverage Determination. Clinical coverage criteria, exclusions, documentation requirements, and any frequency limits are specified in the LCD; this document does not replace those coverage rules.

"HLA-DQB1*06:02 genetic testing may be billed when ordered for evaluation of narcolepsy per the clinical coverage criteria in the associated Local Coverage Determination (LCD)."
