This policy provides billing and coding instructions for NRAS genetic testing: report the appropriate CPT and ICD-10-CM codes, submit 1 unit of service, and include the correct DEX Z‑Code™ identifier adjacent to the CPT code. Place the DEX Z‑Code in the specified claim fields for Part B (Loop 2400 or SV101-7 for 837P; Item 19 for paper) and Part A (Line SV202-7 for 837I; Block 80 for UB04).
