Billing and Coding: MolDX: NRAS Genetic Testing

This policy provides billing and coding instructions for NRAS genetic testing: report the appropriate CPT code and ICD-10-CM diagnosis, and enter 1 unit of service. Include the appropriate DEX Z-Code™ identifier adjacent to the CPT code in the specified comment/narrative fields for Part B (Loop 2400/SV101-7 for 837P or Item 19 for paper) and Part A (SV202-7 for 837I or Block 80 on UB04).
