Polysomnography (PSG) is billed as PSG only when sleep is recorded and staged and the study is directly attended by a qualified technologist; split-night studies should be billed with CPT 95811 (inclusive of 95810). Do not bill separately for EEG/EOG/EMG or additional sleep-parameter monitoring when using PSG codes 95782, 95783, 95808, 95810, or 95811; maintain complete, legible medical records including physician orders (CFR 42 §410.21), ordering NPI on claims, and proof of accreditation/credentialing per LCD L33405.
