Billing and Coding: ProMark® Risk Score

This billing guidance requires ProMark® assays to be reported with CPT code 81479 and ICD-10-CM code C61 (malignant neoplasm of prostate). Claims must list one unit of service and include the appropriate DEX Z-Code identifier adjacent to the CPT code in the specified electronic or paper claim fields for Part A and Part B.

"ProMark® assay service may be billed when the patient has a diagnosis of malignant neoplasm of prostate (ICD-10-CM C61)."
