Billing and Coding: MolDX: Molecular Diagnostic Tests (MDT)

This policy provides billing and coding instructions for reporting MolDX Molecular Diagnostic Tests: submit the appropriate CPT code with one unit of service and include the corresponding DEX Z-Code identifier adjacent to the CPT in designated comment/narrative claim fields. For Part B use Loop 2400 or SV101-7 (837P) or Item 19 (paper), for Part A use Line SV202-7 (837I) or Block 80 (UB04); do not add extra characters on SV101-7.
