SEP testing should be billed using CPT codes 95925, 95926, 95927, or 95938; intraoperative continuous monitoring during spinal surgery may be billed with CPT 95940 or HCPCS G0453 in conjunction with SEP codes when one-on-one monitoring is provided. Quantitative Sensory Testing (hand-held QST) is not SEP testing and is considered part of evaluation and management (not separately billable); intraoperative monitoring by a technician is not separately reimbursable and multiple/bilateral modifiers do not apply. Detailed, legible medical record documentation (including test recordings, physician interpretation, nerve-specific latencies, and evidence the results influenced treatment) must be maintained and available upon request.
