This billing/coding guidance complements MolDX LCD L36400 for genetic testing for hypercoagulability/thrombophilia (Factor V Leiden, Factor II prothrombin, MTHFR). Claims must report the appropriate CPT code with exactly 1 unit of service and include the appropriate DEX Z-Code identifier adjacent to the CPT in the specified Part A or Part B claim fields; do not add extra characters on SV101-7.
