Billing and Coding: MDS FISH

This policy provides billing and coding guidance for MDS FISH (ISH) testing: use CPT 88271-88291 when ISH is an ancillary cytogenetic analysis for oncologic or inherited disorders, and use CPT 88365-88377 when a pathologist performs ISH as an adjunct to surgical pathology or cytopathology. Do not use 88365-88377 for pathologist-performed ISH that is ancillary to cytogenetic studies. Claims must include the appropriate CPT, one unit of service, and an ICD-10-CM code; DEX Z-Codes are optional but, if provided, must be placed in specified claim fields.
