Policy Summary
CPT 64450 covers direct nerve block injections (anesthetic and/or steroid) into peripheral nerves when performed by a qualified practitioner for conditions such as peripheral neuropathy. Subcutaneous injections into surrounding tissue are not billable under 64450, and billing must comply with the related LCD medical necessity requirements; complete, legible medical records identifying the provider and supporting the diagnosis and billed code are required.
