Non-invasive peripheral arterial vascular studies are covered only when medically necessary and when results will be used in patient management, with the medical necessity documented. Routine ABI-only assessments, preventive/screening studies, and studies without documented clinical utility are not covered; generally studies should not exceed one per year (exceptions for inpatient, ER, preoperative for bypass [one allowed], immediate postoperative indications, and documented clinical situations such as abnormal physiologic tests, trauma, thromboembolism, or aneurysm).
