Policy Summary
Non-invasive peripheral venous and hemodialysis access studies are covered when the study results will be used to guide clinical management and when medical necessity is documented. Routine outpatient studies are generally limited to once per year (inpatient and ER visits excluded), with specific allowable exceptions such as one preoperative hemodialysis access scan, one limited postoperative study within 72 hours of saphenous vein ablation, and combined duplex/physiologic testing when clinically indicated and documented.
