Policy Summary
SCODI is covered when medically reasonable and necessary to manage retinal conditions undergoing active treatment in accordance with LCD L35038. Utilization limits include no more than one exam per 28-day period for active retinal management, a maximum of two reports per year for CPT 92132 and 92133, and a maximum of one report every two months for CPT 92134. All supporting documentation must be legible, include patient identification and provider signature, justify the ICD-10-CM diagnosis codes, and be available upon request.
