Policy Summary
Cystatin C measurement (CPT 82610, 1 unit) is covered for adult patients when documentation shows a creatinine-based eGFR of 45–59 ml/min/1.73 m2 without markers of kidney damage and when creatinine-based estimates are believed inaccurate and the result will affect clinical decision-making. Claims must include appropriate ICD-10-CM support, a physician requisition, complete and legible medical records with provider signature, and documentation of how the test result was used in the patient's plan of care.
