Coding for MDS FISH depends on the clinical context: use CPT 88271-88291 when ISH is ancillary to cytogenetic studies for oncologic or inherited disorders, and use CPT 88365-88377 when ISH is performed by a pathologist as an adjunct to surgical pathology or cytopathology. Submit the appropriate CPT and ICD-10-CM codes with 1 unit of service; a DEX Z-Code is not required but, if included, must be placed adjacent to the CPT code in the specified claim fields for Part A and Part B.
