Billing and Coding: MolDX: Inivata™, InVisionFirst®, Liquid Biopsy for Patients with Lung Cancer

This billing/coding guideline specifies how to report the MolDX Inivata InVisionFirst–Lung liquid biopsy under LCD L37897: use CPT 81479, report 1 unit of service, and choose the appropriate ICD-10-CM code for the patient's lung cancer. The claim must include the appropriate DEX Z-Code adjacent to CPT 81479 in the specified comment/narrative fields for Part B (Loop 2400 or SV101-7 for 837P; Item 19 for paper) and Part A (Line SV202-7 for 837I; Block 80 on UB04).

"InVisionFirst–Lung (Inivata) liquid biopsy is to be billed for patients with lung cancer consistent with MolDX LCD L37897."
