Services for lower‑extremity varicose veins are coverable only when the record documents symptomatic varicose veins, failure of an adequate (at least 3‑month) trial of conservative management, and any medically necessary tests confirming vascular anatomy; services lacking such documentation or medical necessity will be denied. Claims must include appropriate ICD‑10‑CM codes, follow HCPCS/CPT and Correct Coding Initiative edits, and supporting documentation must be available to Medicare on request.
