Policy Summary
This billing and coding guideline complements the LCD for vitamin D assays and requires accurate ICD-10 coding, ordering physician name/NPI when applicable, and medical-record justification of the selected test. Procedure codes may be subject to NCCI/OPPS edits, composite sources of 25‑OH vitamin D should not be billed separately, and repeat testing for levels 20–50 ng/mL in clinically stable patients requires documented medical necessity.
