Mohs Micrographic Surgery (MMS) is covered for complex or ill-defined malignant skin neoplasms—particularly at critical facial sites—when 100% margin examination and a single surgeon acting as both surgeon and pathologist are performed. Facilities must hold an appropriate CLIA certificate to bill MMS; separate same-day biopsy/pathology payment is allowed only with proper modifiers and documentation (including no biopsy of the MMS lesion within 60 days or a different lesion), and extensive operative, histologic, and counseling documentation is required to support billing.
